Your medical records will be stored electronically — for 10 YEARS — with Morgan Records Management. Requests are fulfilled in the order they are received and may take up to 30 days to complete.

It is with mixed emotions that we post this notice. We have closed Reliance Medical Associates. Any scheduled
appointment after May 26, 2023 has been cancelled. There are many excellent primary care physicians in our community. I recommend that you contact your insurance company to find out which providers are on your plan.

Notes For Patients
The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than seven years after the last date of services you received from this Practice, although most records will be held for at least 10 years from the date of last service, depending upon applicable state and federal record retention requirements.
